Patient Portal

OSS Health has implemented a Patient Portal to enable you to access your medical information in a secure manner via the internet. Following are just some of the functions that are available via the Patient Portal:

  • Schedule appointments
  • Check upcoming appointments and request changes if necessary
  • Update demographic, medication and insurance information
  • Update current list of medications
  • Send non-urgent messages to the physicians and their teams 24 hours a day/7 days a week
  • Request a medication refill
  • Add or remove allergies
  • View lab results
  • Request and receive work notes and disability forms

To active your portal, please request an activation code via e-mail to it@osshealth.com. After you receive an activation code, click here to access our Patient Portal. Click on new user and then click on Activate Patient Portal Account. You will need the following information:

  • Name
  • Date of birth
  • E-mail address
  • Create a login and password
  • Enter activation code provided by OSS Health
  • Update security information
  • Select an ID and phrase.
